Under the supervision of a registered nurse, the home health aide/homemaker is available to provide a variety of services in the home setting, including:

• Conversation and Companionship

MEDICATION MANAGEMENT

A registered nurse is available to develop a coordinated medication management plan with the patient. The nurse can pick up prescriptions, answer any questions the patient has about these medications, and ensure compliance with an organized medication schedule.

SKILLED NURSING CARE

For clients with more advanced health care needs, registered nurses can provide necessary health care services, including:

Post-discharge care management (from hospital, rehab or nursing home stays)

Chronic health care management, such as:
